The image was scanned from Phillip's solo CD Protocol, which he promoted on a clinic tour. (I believe the CD is out of print. Please don't write me and ask where you can get your own, because I don't know, and so I'm just as likely as not to name some obscure mail-order reference you'll spend weeks tracking down, only to be disappointed.)
